Form to Fill out for Head2Toe- FREE TENNIS SHOES for CURRENT(2024-2025 school year) PreK-3rd Grade Students!

Please fill the below form out for each child that your family has at LCES in Prek-3rd Grade!

We are so thankful to partner with First Baptist Church of Lenoir City for a second year to see that our students are getting NEW shoes to start a new school year. 

We need your help to make sure each student enrolled at LCES during the 2024-2025 school year in grades PreK-3rd grade gets the right size.  We are requiring parents to complete this form to insure each child gets the shoes they need.  

To receive the shoes for your child:
1. The child has to be enrolled in Pre-K-3rd Grades at Lenoir City Elementary school (2024-2025 School Year).
2. THIS FORM has to be completed and returned to the Family Resource Center by Thursday, April 17,2025. 
3. A parent/guardian of the child must pick-up their child's shoes at the summer event at First Baptist Church of Lenoir City will hold. The Head2Toe shoe distribution will be on Saturday, August 9th. Time TBD.

Estamos muy agradecidos de colaborar con la Iglesia First Baptist de Lenoir City por segundo año consecutivo para asegurarnos de que nuestros estudiantes reciban zapatos NUEVOS para comenzar un nuevo año escolar.

Necesitamos su ayuda para asegurarnos de que cada estudiante inscrito en LCES durante el año escolar 2024-2025, desde preescolar hasta tercer grado, reciba la talla correcta. Solicitamos a los padres que completen este formulario para asegurar que cada niño reciba los zapatos que necesita.

Para recibir los zapatos para su hijo/a:
1. El niño/a debe estar inscrito en preescolar hasta tercer grado en la escuela primaria de Lenoir City (año escolar 2024-2025).
2. Este formulario debe completarse y devolverse al Centro de Recursos Familiares antes del jueves 17 de abril de 2025.
3. Un padre/guardian del niño/a debe recoger los zapatos de su hijo/a en el evento de verano que se realizará en la Iglesia First Baptist de Lenoir City. La distribución de zapatos "Had2Toe" será el sábado 9 de agosto. Hora por determinar.
